Please contact Dr. Combs-Lane to schedule an intake evaluation. You will be provided a link to your Client Portal in order to complete online forms prior to your visit.

I do not accept insurance. You will need to self-pay for services.
Please check with your insurance company for your out-of-network coverage. I can provide a superbill to you.
